Registration/payment deadline is January 15, 2010. Please make checks payable to St. Mary’s Foundation and mail along with a copy of this form. A confirmation will be mailed to you.  

 

Cancellations/Refunds: Cancellation notices and requests for refunds must be made IN WRITING no later than January 1, 2010 to be eligible. NO refunds will be granted after January 1, 2010. If you are unable to attend, you may transfer your paid registration to another person who is not already registered.
